Narrative

An employee operating a stand-up forklift removed a pallet from a storage rack. His left foot was hanging over the rear of the forklift as he backed away from the rack. While reversing, he struck the storage rack behind him, injuring the toes on his left foot. He was wearing steel-toed shoes at the time. His second toe was amputated.
