Narrative

Employees were dismantling a scaffold and were handing down a piece of plywood, which was 4-feet by 6-feet, when it slipped from a coworker's hands and fell 50 feet. The plywood struck the injured employee in the left foot behind the steel-toe shoe. His left toes were amputated.
