Narrative

Two employees were preparing to weld a 6 inch pipe. The anchor bolts that were holding the pipe came out of the concrete. The pipe fell and struck the ladder on which the employee was working. The employee was thrown 4 feet off the ladder to the concrete below striking his head. The employee suffered fractured ribs, a fractured eye socket, and minor bleeding to the brain.
