Narrative

An employee had his right foot on rollers to walk around a pallet that was in the walkway. When the employee placed his right foot in the rollers, the rollers turned. This caused the employee to fall backwards. His left foot struck the pallet resulting in a dislocated ankle and a broken tibia.
