Narrative

An employee entered an oven from a side access door located 12 feet aboveground to clean and clear the rollers of the coating machine. When the employee was in the oven, she stepped onto a pressure release trap door, causing the door to open. She fell 12 feet to the ground below, sustaining multiple fractures to her right ankle and possibly lower back fractures.
