Narrative

An employee was performing preventative maintenance on piece of conveyor. He reached across and lifted up on the belt. His back elbow crossed the photo eye, activating the belt. His finger was caught between the end of the conveyor and the end guard. When he pulled his hand back, he degloved the right ring finger resulting in medical amputation.
