Narrative

On December 17, 2015, at approximately 9:00 AM, an employee was empting a bag of recyclable material into a grinding machine when the bag became lodged into the equipment. The employee tried to free the bag from the equipment and he was pulled into the equipment. The employee suffered broken bones and damage to his skin on the right wrist and left arm.
