Narrative

An employee was preparing to conduct preventative maintenance activities on an HVAC unit on the roof of the building. He took the front panel off the unit. He slipped/lost his footing, causing his hand to enter the unit and get caught in the desiccant belt wheel. His left index fingertip was amputated.
