On January 25, 2016, at approximately 4:30 p.m., an employee was completing rig service on the top drive. The service door was open and lowered with a winch. The employee was positioned on the service door, approximately 8 to 10 feet above the ground, and wore fall protection. The remote for the winch developed an electrical short, causing the winch to re-spoil. This caused the door to raise back up. The employee was pinned between the service door and the top drive, resulting in two hairline hip fractures.
