Narrative

An employee was unhooking chain slings on a crane moving via remote control. Hooks from the overhead crane, which was being operated remotely by a coworker, caught a 6-foot-tall bundle of steel stacked behind the employee and pulled it over onto the employee's leg. The employee's leg broke.
