Narrative

On March 13, 2016, an employee was putting a belt (48" wide by 17' long) on the screener. The employee was on a ladder three rungs up. When he attempted to pull on the belt, the ladder went out from underneath him and he fell to the floor. He suffered a compression fracture to his lumbar vertebra.
