Narrative

An employee was feeding material into the moulder during the setup process. The employee fed a thin piece of material into the moulder and then thought it was jammed in the machine. When she went to the operator to point out where it was jammed, her glove was pulled into the cutter head and her index and middle fingertips were amputated.
