Narrative

An employee entered a bag holder to access a bag placer carriage and monorail where a bag had fallen. While the employee was kneeling below the monorail to pick up the bag, the bag placer carriage engaged, pinching the employee's head between the carriage and monorail. The employee suffered head lacerations that required sutures, as well as contusions on both sides of the head and nose.
