Narrative

An employee was guiding an 8" PVC water pipe that was attached by a synthetic sling to the bucket of an excavator. The pipe was being set in place when the sling slipped from the pipe. The pipe rolled on the ground and struck the employee in the leg, fracturing the employee's upper thigh bone.
