Narrative

As an employee was removing a tube from a box while standing on a stair ladder, the box came off the shelf, causing the stair ladder and employee to fall 9.5 feet to the lower level. The employee landed on a box and broke his left hip, left wrist, and several left-side ribs; cut his spleen; and fractured his back vertebrae.
