Narrative

An employee hoisted the lubricator up off a well head using a winch and was inspecting the tool stream. The winch malfunctioned and continued to raise the lubricator up which was 8 feet long. It was raised approximately 15 feet off the ground. The cable snapped and the lubricator fell striking the employee and causing a laceration to his chest approximately 6-8 inches long and 1 1/2 inches deep.
