Narrative

An employee and a coworker were lifting up a manhole to see the direction of a pipe. The lid was lifted and held vertically. Some dirt started to fall into the manhole, and the employee used his hand to brush it away so it did not fall into the manhole. The lid slipped off the tool and smashed the employee's left thumb between it and the casting, amputating the end of his left thumb.
