Narrative

On 6/5/16, at approximately 12:15 p.m., an employee was climbing onto a mobile scaffold to install drywall mud. The employee was ascending an add-on scaffold ladder designed for safe scaffold access when his foot was caught on the second rung. He then fell less than 4 feet, fracturing three vertebrae in his neck.
