Narrative

An employee was trying to unjam a drive shaft on a vacuum pump with a pipe wrench. The drive shaft unjammed, causing the pipe wrench that was still on the drive shaft to spin and strike the employee in the head and eye socket area. The employee suffered a head laceration and a skull and eye socket fracture.
