Narrative

An employee was cleaning out chunks of wet PVC resin from the side of a mixing machine. While he was attempting to remove the bolted-on steel plate, the plate (which was under pressure) blew out and struck the employee on the right hand and dorsal forearm. He sustained three broken fingers on his right hand, a torn-off fingernail, and lacerations to his right thumb and left dorsal forearm.
