An employee was on his ladder, replacing a mid-span drop. His ladder was resting on the strand (cable line from pole to pole). When he cut the line off at the mid span (toward the middle of the strand), quick release in tension caused him to fall from his ladder to the ground. He fractured his pelvis and chipped a bone in his ankle.
