Narrative

An employee was working on a 15-foot scaffold with zip ties and was bent over conducting work. When the employee raised up from working with the zip ties, the employee suddenly passed out and hit the floor of the scaffold. The employee then rolled off and fell 15 feet to the ground, fracturing ribs.
