Narrative

On 8/8/16, an employee was cutting metal on a shear. The employee's gloved hands became caught in the metal hold-down pads that push the metal into the shear. The employee suffered a right-hand middle fingertip amputation, as well as left-hand middle and index fingertip amputations.
