Narrative

An employee was holding a piece of plywood in his right hand by the steel structure of a conveyor. A forklift was returning a dumpster to that area and the forklift struck the plywood pressing the employee's finger between the plywood and the steel structure of a conveyor. The tip of the employee's right ring finger was amputated.
