Narrative

An employee was dismantling a scaffold when the pin from the toe board came out, lifting the guardrail up. The employee went to grab the guardrail as it was falling and fell with it. The employee fell approximately 20 feet to the ground below, fracturing an arm and cheekbones and suffering numerous contusions.
