Narrative

An employee was working from a cable tray 15 feet above the ground. The employee was utilizing a fall protection harness in conjunction with a PFAS. As he tried to move from one location to another, he fell to the floor, shattering his right heel and fracturing his left ankle. His lanyard was not tied off at the time of the incident.
