Narrative

On September 16, 2016, an employee was attempting to remove a rock from a running conveyor belt. When the employee attempted to retrieve the rock, the conveyor grabbed his glove and pulled his arm into the drive pulley assembly. His right arm was crushed and had to be surgically amputated.
