Narrative

On October 4, 2016, an employee was positioning a mold that was to be lifted by a gantry. The employee's hand was on one of the chains attached to the mold. When the chain was tightened, the employee's left thumb was caught and partially amputated by the chain (tissue and nail loss; no bone loss).
