Narrative

An employee was in an aerial lift washing down plant equipment with a fire hose. The fire hose became tangled in the aerial lift causing it to tip over and the employee fell 16 feet to the ground. The employee was hospitalized for a head injury, a collapsed lung and a laceration on the throat.
