An employee instructed a coworker to reverse a forklift so he could use the back portion as a work platform. He then cut a piece of 1/4" manila rope to use as a tagline around the protruding end of a pipe. He attempted to tighten the rope, but when he cinched down, the 1/4" manila rope broke, causing him to fall face first into gravel. He suffered two fractured left eye bones and three fractured ribs.
