Narrative

An employee was assisting with the extraction of a 4-foot by 24-foot steel plate from an excavation. As the steel plate cleared the top opening of the excavation, it rotated and struck the employee in the right side, causing a serious contusion and small tear in the kidney that required hospitalization.
