Narrative

An employee was attempting to fix a belt drive in a casing machine when a coworker plugged the machine back in, causing the belt drive to engage. The employee's right index finger was then caught in a sprocket and de-gloved, resulting in a medical amputation to the first joint. The equipment was unguarded at the time of the incident.
