Narrative

On December 13, 2016, at approximately 2:30 p.m., an employee was climbing an extension ladder to access the roof of a residence when he fell approximately 12 to 15 feet to the ground, fracturing his back. The top and bottom of the ladder were secure at the time of the incident.
