Narrative

An employee was using a torch to cut a 10 inch diameter pipe for removal on top of a pipe rack approximately 30 feet high. The employee's fall protection equipment was unhooked to move down the pipe. The employee lost his balance and fell 30 feet resulting in two broken wrists.
