Narrative

An employee was installing an arch pipe in a driveway. After the employee released the mechanism on the hoisting apparatus that held the pipe, the pipe fell on the employee's finger, pinching it between the pipe and the concrete driveway. The employee's right ring finger was amputated at the first joint.
