Narrative

The employee was moving a manifold via three portable pipe stands when one of the stand's casters got stuck in a divot in the cement floor. As the employee attempted to push the manifold from the first stand, the weight of the manifold became unevenly distributed causing the manifold to fall from the pipe stands. The employee's finger was caught between the pipe stand and the manifold resulting in the amputation of the employee's first digit of the right thumb.
