Narrative

On 3/15/17 at approximately 9:45 PM, an employee crawled under a bag machine to place a bucket in the paper catch to capture dripping glue while the machine was running. The employee's hair was caught in a moving shaft in the spot paste unit resulting in a scalping injury.
