Narrative

An employee was transferring a chemical (alkali) from a bulk tank to a smaller tank. The smaller tank overfilled and chemical spilled onto the floor. The employee slipped and fell on top of the chemicals resulting in chemical burns to the left side of his hip, the back of his leg and his face.
