Narrative

An employee was adjusting the jib lock at the side of the main boom on a 55-ton boom truck. He loosened the lower securement bolts. While loosening the upper bolts one of the lower bolts came out and the jib slipped off of the locking plate. It dropped a couple feet until resting on the cab and struck the employee on the face and left shoulder.
