On April 3, 2017, an employee was adjusting a roll of plastic material being fed into a thermoforming machine while standing on a small metal grated platform. The employee stumbled and the employee's hand was caught in a pinch point where the machine chain drew the material in, resulting in amputation of the right middle fingertip.
