Narrative

An employee was checking the ID of a pipe for smoothness. While she was checking the inside of the pipe with her thumb, the machine cycled pushing the pipe back against a back stop and trapping her thumb between the back stop and the wall of the pipe. The tip of her thumb was amputated.
