Narrative

On April 15, 2017, at about 2:30 a.m., an employee was cleaning a primer day tank when the agitator in the tank started to move. The hose the employee was using to clean the tank wrapped around the agitator, causing her right arm to be caught in the moving agitator. She suffered a fractured right arm and right pinky.
