Narrative

An employee was checking a belt in an HVAC unit on a roof. The unit was shut off and locked out. However, 30 mile per hour winds caused the fan to move and pinch the employee's left pinky finger between the belt and the pulley, resulting in amputation of the left pinky fingertip.
