Narrative

An employee was lowering a billet to the floor level using a chain sling basket. A sway developed in the load, and the end of the billet (farthest from the employee) landed on the already staged billets. Slack then developed in the chain, and the billet shifted and slid off the staged billets. The end closest to the employee landed on his foot, crushing his big toe and second toe.
