Narrative

An employee climbed a portable extension ladder (approximately 14 feet) to inspect and photograph a water line valve. The ladder slipped and fell, causing the employee to hit the concrete floor below. The employee suffered a broken left wrist and shoulder blade, a sprained or broken left ankle, and a nose injury.
