Narrative

On Thursday, June 1, 2017, at approximately 6:55 AM, an employee was performing a change-over on a machine. He was holding a plate inside the machine with his left hand when the machine started and caught his left hand, partially amputating his left middle finger and lacerating his left index and ring fingers.
