Narrative

An employee was painting the front section of the first floor entryway of a three-story split plan house. He attempted to access the exterior wall between the first and second floor using a 6-foot step ladder on top of a 3:12 pitched metal roof. He slipped and fell about 4 feet from the ladder onto the metal roof, fracturing his right hip and requiring hospitalization.
