On June 28, 2017, at approximately 11:30 a.m., an employee was on a ladder cleaning an empty metal bin when he fell into the inside of the bin, suffering a cut on his arm, cracked rib, chipped hip bone, cracked pelvic bone, and cracked upper arm/shoulder bone.
