Narrative

An employee attached a chain sling to a manual frac valve that was to be lifted by an overhead crane. The crane operator lifted the valve, noticed it was not aligned properly, and lowered it to be corrected. As the crane lowered the valve, it slipped from the chain sling and struck the employee's left foot, crushing the large tow.
