An employee was operating a press when a part and the draw pad became stuck on the bottom of the die. The draw pad was under extreme pressure. As the employee tried to pry the part off of the die, the part released, causing the draw pad to spring up. This broke the bolts containing the draw pad, and the draw pad hit and fractured the employee's wrist.
